The Sioux Falls School District policy requires Proof of Immunization, A certified copy of the birth certificate or an affidavit issued by the Department of Health affirming the original birth certificate has been deemed unattainable.

If the immunization requirements present a financial difficulty, immunizations are available at:

    Cost is based on income. Medicaid is welcomed.

  • Avera McKennan Health Care Clinic: (605) 322-6800
    This is a free clinic designed only for those who have no health care insurance or private funds

  • Serve all students

  • Help students stay in school to receive needed care rather than leaving campus

  • Remove barriers such as lack of transportation or insurance

  • Be accessible with a convenient location in a familiar setting

  • Provide High-Quality Primary care with a strong emphasis on prevention

  • Provide Chronic Disease Management with an effort to decrease student absences

  • Connect students with community providers when the diagnosis is beyond the scope of FCH
