HOURS: Monday and Tuesday from 9:00 am - 3:00 pm; Wednesday - CLOSED; Thursday and Friday from 9:00 am - 3:00 pm.
LOCATION: 201 West Ave, Door "M" at the Community Engagement Center located in Axtell Park.
SIGN-IN AND COMPLETE: last name; household number: total number of kids, adults (18+), and elderly (60+); name of schools where kids are attending.
GUIDELINES: one visit per month per household; one member of the household MUST be present; check-out with staff.
